As part of a college assignment, I need to find out opinions on a particular subject. I decided a web-based questionnaire/survey would be the best data collection method for this, but I have no idea where to start.

Googling only turned up a load of results for free software for creating them, which is not what I want. I want to do it myself.

I know next to nothing about forms, and collecting data from them, so any help would be appreciated.

Ideally, the data collected would be stored in a database so I wouldn't have to collate it all myself, since I'll be aiming to get at least 100 filled in, but that would be considered more of as a helpful extra than a necessity.

As long as the form data can be collected and returned to me in some way, I'm happy.

(Sorry if this is in the wrong place, it seemed the most appropriate)
